Record commit/release batches, cross-module or long deliveries, important root causes/red-zone changes, large governance changes, material history corrections and package publication state.
Ordinary uncommitted edits, isolated small fixes/tests/styles/refactors and metadata/discussion-doc edits do not require new logs. Large-delivery execution still records progress. Reuse a related active batch instead of creating one record per correction; explicit user requests override this gate.
Thought/design/plan/PRD documents stay in their own directories; logs link them.
The iteration mechanism requires dated anchors and dotted role suffixes for thought, design, plan, and loop deposits:

Create a new iteration only when the goal has changed, impact expanded, a new independent batch started, a release/commit loop closed, or the user explicitly asked for separate traceability.
If a tiny new iteration was created by mistake, merge its useful content back into the related iteration and remove the mistaken directory.
