Do not repeat the diff, PR text, or prior comments. Group related existing-feedback threads into one concise entry. Omit empty sections and boilerplate.
Make each finding easy to scan: state the problem, its real effect, and the
smallest useful correction. Include only the detail needed to understand and
act on it. Use precise code names and path:line references instead of
background explanations. Explain an unfamiliar technical term before using it.
Do not soften actionable findings with vague language such as "might be worth".
